You may not have Permanent Make-up if you are pregnant or nursing. You must be 18 years of age.

If you have any existing medical conditions or have had any cosmetic surgeries on your face within the last year, you must have a doctor's release before any Permanent Make-up procedure.

The release must be faxed from your doctor to 831-763-2552 attn: Janette. It must be on your doctor's letterhead, stating your name and the procedure requested, followed by the words "by Janette Archer."

You must notify Janette Archer of any change that occurs post-consultation before a procedure is performed.

Botox, Restylane or any other injectables require twelve weeks before your next Permanent Make-up session may be performed; initially, before the follow-up appointment, or any other additional sessions in the future with Janette Archer. Any injectables less than twelve weeks before will require a doctor's OK.

You may choose to have a patch test for skin sensitivity to pigment. A patch test will require that you wait 8 days before having your Permanent Make-up procedure. Although extremely rare, some individuals may experience an allergic reaction. This test must be scheduled at time of consultation.

If you have ANY history of cold sores or herpes on your mouth, you must get a prescription from your doctor before the procedure to prevent an outbreak.
Remember:
